Register your School to Participate in the 2010 IYS

“The data gathered from the Illinois Youth Survey helps the Illinois State Board of Education document overall trends and specific issues related to emerging substance abuse and violence, which often impede a conducive and productive learning environment for all Illinois students. The survey also measures several key risk and protective factors surrounding substance abuse. Your school's participation in this survey will help us give communities the information they need about local, state and regional trends in substance abuse. Therefore, I strongly encourage your participation in the spring 2010 Illinois Youth Survey.”

Thank you for your interest in participating in the 2010 Illinois Youth Survey! We look forward to helping your school obtain current, relevant data about the many issues facing youth today, and looking at how your school’s data translates into several key risk and protective factors that influence drug use and other behaviors.

When you click on the registration link below, you will be asked to provide identifying information about your school, contact information for the person coordinating the survey, and grade level enrollment numbers for the grade levels participating in your school (6, 8, 10, and 12.) With this information at-hand, completing the registration survey using Zoomerang should only take about 5 minutes.

After you register, an email message will be sent to the named contact person within one week to confirm that your registration has been received and to enable you to verify that we have processed your information correctly. If this email has not been received within a week and your school has been registered, please contact Jessica Thomas at 309-451-7849 or Donna Prather at 866-497-9727.

Tips to completing your school’s registration:

1.

You must submit a separate registration for each school you wish to register.

2.

Please be sure to complete all the information requested, including the current enrollment numbers for participating grade levels.

3.

An email address for the school contact person is essential so we can send your confirmation email and contact you with any questions we may have.

4.

Shipping addresses must be a street address, not a PO Box number.

5.

Your survey date is also essential, as shipment of your survey materials is based on the date you plan to survey. Without an approximate survey date, no survey materials will be shipped.
