Survey Content

There are three different IYS forms: one for 6th grade, one for 8th grade, and one for high school (10th and 12th grades). The 8th grade survey includes all of the 6th grade questions but adds a few more. The high school survey includes nearly all the 8th grade questions, but adds a few more. The sections on each survey are: Demographics, Community, Drug Use, Physical and Other Injury, Individual/Peer, Nutrition/Fitness, School, and Family. Schools can choose to get the paper survey or sign up for online administration. Both the paper and online surveys offer an option for schools to add up to fifteen local, user-defined questions to the survey. The results for these questions are included in the school’s initial report by question number. Please refer to “Developing Additional Questions for the Illinois Youth Survey” for more information.

By clicking on the appropriate form below, you can view the 2010 survey questions or survey questions from 2008. PLEASE NOTE: To participate in the state-sponsored survey, including tabulation of results, schools must use either the printed forms provided (free) to Illinois schools by the survey project or the online version. Contact the IYS Office to obtain surveys. Surveys printed from the documents below cannot be scanned, and may invalidate later administration of the state-provided 2010 survey.
