Survey Content

There are three different IYS forms: one for 6th grade, one for 8th grade, and one for high school (10th and 12th grades). The 8th grade survey includes all of the 6th grade questions but adds a few more. The high school survey includes nearly all the 8th grade questions, but adds a few more. The sections on each survey are: Demographics, Community, Drug Use, Physical and Other Injury, Individual/Peer, Nutrition/Fitness, School, and Family. Schools can choose to get the paper survey or sign up for online administration. Both the paper and online surveys offer an option for schools to add up to fifteen local, user-defined questions to the survey. The results for these questions are included in the school’s initial report by question number. PLEASE NOTE: To participate in the state-sponsored survey, including receiving tabulated results, schools MUST use the printed forms provided by the survey project or the online version. Survey forms are provided free to Illinois schools. Contact the IYS Office to obtain surveys.

Please do not print the documents below and use them to survey students as they cannot be scanned. Using forms other than those provided by the survey project may invalidate later administration of the state-provided 2010 survey.
